You should have already completed your new hire enrollment period for medical, dental and vision in Hoshonti, however, your new hire enrollment for Permanent Whole Life insurance needs to be made here.

RESPONSE REQUIRED - As a benefit eligible associate, you are required to sign in, review the voluntary permanent whole life insurance with long term care benefits, and accept or decline coverage. You will receive a confirmation statement via email.

This is your opportunity to protect your family's financial security at an affordable rate in the event you need long-term care services or at death. The maximum death benefit for associates is $150,000 if you are age 50 or younger, $60,000 if you are age 51 or older, with a maximum $3,000 LTC monthly benefit.

Benefits are received tax-free and individual participation is available for you, your spouse, children and grandchildren.

Premiums are paid by direct bill to your credit card or debit card beginning the 5th of the month that your policy becomes effective. This benefit is 100% portable when you leave at no increase in cost.

Acceptance is guaranteed which means no health questions asked, within plan parameters.
